Live completely fulfilled lives in their homes.
Funding for Supported Living is provided through the Local Council & NHS
Collaborative approach
Involves the person, families or authorized person in making decisions.
Individualized support
Creating an authentic and tailored personal support plan outlining the type of support they receive.
Progressive
Continuously working towards independence in order to promote choice and control in their lives.
Personal grooming and hygiene
Bathing or showering
Toileting

Light meal preparations and housekeeping
Medication assistance program
Participation in leisure activities
Depends on the individualized needs assessment and may include:
Occasional visits from a Support Worker
Flexible supports during the day, evening, overnight and /or weekends
24/7 with waking or sleep in nights
1:1 packages
Shared staffing for shared accommodations
